Sir I am 38 year old male suffering from hypertension from last 13 years prescribed Zitelmi H & Concor AM 5 from last 10 years due to which I feel I have gained around 30 KG of weight my b12 is very low at 80 only, condition is very dizzy, lost my stamina week eye sight etc, please advise are all these side effects of medicines I am consuming or anything else.

Weight may be from poor diet. Reduce sugar jaggery alcohol and starchy foods. Increase traditional health foods like butter fish eggs nuts etc. You are taking very strong combination drugs, they can add to weakness if dose is excessive.
